Every Approvals on Change are getting triggered twice

Hello,

Currently in our normal change workflow, we have three approvals (i.e) Technical, One customized group approval, CAB Approval. Whenever the approvals are triggered, It's triggering twice. In the Approval section, I can see the approval types says whether it is "technical/ CAB". But for the duplicated ones, I couldn't see any names for the approval.

Any guidance is much appreciated.

You will have to share more information. How are they set up? Is it a flow/workflow? How are they triggered? What did you already check on/troubleshoot?
