How can I get the incident form to show up in Service Portal Designer?

Hi, 

I have added an Icon Link widget to my main page of the Service Portal I am creating. I would like the link to take the person straight to the incident form to fill in. However, I cannot seem to get the config right to show the form. 

 

Any advice please?

Then you just need to select type as catalog item & make sure you select a cat item & not a record producer.

This is kindof what I need! 

 

So this is showing me the incident form how I would see it as an ITIL user. I have another form which the customer sees which is in service catalog - catalog items - called 'create incident'. 

 

Would you know how I can show this please?
