You cannot have it differently for some catalog items and something for other catalog items.
change the config in My request filter
Change the "Primary field to display" field, but remember you can't dot walk there to Item->Short Description
To handle that you will have to clone the OOTB "My Requests" widget, update the server side script and then add this widget again to the portal page.

still the issue is==Our primary field is short description, but the portal still displays the catalog item name. Could you please help with the widget changes needed to display the user-entered short description on the portal side?"
