Report to capture how many times the assignment group changed

Can anyone please help on the below requirement

I want to capture the details of requests changing from one assignment to another in last 6months

Basically it should display

1. The old assignment group which it was assigned

2. Changed to which

3. How many times it changed.

Checkout Metrics - Definitions, I think 'Assignment Group' is an OOTB definition.  You should be able to report off the Incident Metric table.

This function is already available using Metrics, there is already an out of the box metric definition created that captures assignment group changes for an incident.

If you go to the incident form and "Configure Related lists" and add the "Metrics" list.

You will see this, each time you change a  group, it captures the group and how long it was with them.

Thanks for your reply. I have checked that.

But i need this to be performed on Record producer requests, like request items. 

Basically for the requests which are re-assigned to different groups in the HR case table.
