Stage: Catalog Item Removed - What Is Causing This?

Hi Everyone,

I have a quick question in regards to some requests that come into the system where the Stage will show a red "X" with the text "Catalog Item Removed." All other information in regards to request is removed (deleted) and the only information available is who opened the request and what time the request was opened.

I have checked the Wiki's etc., and the only thing that SHOULD be causing this is if I remove a Catalog Item while an active workflow item exists when I delete a Catalog Item. The issue is that I have not deleted any Catalog Items - what are possible causes of this error?

These Requested Items might be getting created due to possibility of bad script. Are you seeing any other RITM created at the same time when this RITM is also getting created.

I have noticed that there are 2 out of 5 RITM's that have the Catalog Item Removed Stage with the exact same time stamp. This seems to be an anomaly as it has only happened once.

KB specifies SLA issues, but the same fix works for catalog items also.
