Microsoft Team notification is not working, even after the logs are getting generated

I have created a provider notification on the Incident and Request tables, triggered on Insert/Update when Active = true, with the content type set to Simple and a simple message body.

When I test this, the logs show the updated incident number correctly, but the notification is not being received in Microsoft Teams.

I have already checked the following, but no luck so far:

  1. sys_notification_execution – shows the correct deliverable count
  2. Navigated to Conversational Interfaces → Settings → Microsoft Teams Integration and confirmed that “Enable Notification for all users” is turned ON
  3. Verified that the sn_now_teams plugin is installed and up to date.

Could you please help here. Thank you!!
