Record Creation Error

Hi all,
 I created a mobile application in now agent. When I try To Create a incident record from mobile application. It showing 
"Invalid update record has been deleted or you no longer access to read ".Even though I created application as per guidance. Every Step I followed and Created the function and action Item. what is the problem can anyone please explain .

Raise a HI ticket . They will be able to guide you further .

from where i can raise ticket can you please explain.there is no option for that.

on HI portal , you manager or architect might be having access for that. Tell them to raise it on your behalf
