New assessment metric question not displayed in the assessment survey form

I created a new assessment metric for Risk category but It does not displayed in the assessment survey questions( see screenshots). Can you help me on this please !

Hi Namita,

Facing same issue couldn't access your recorded video. My Issue is I am not able to see the assessment metrics on Surveys & assessments widget on portal. But, I can see on platform view.
