Create a simple feedback survey and make it always available in portal for users to submit

I've a requirement to create simple survey with feedback and experience rating out of 5 star and make that survey available in portal as a header menu so that users can rate there experience on demand, whenever they wish to.

I've created a survey but not sure how can make it available on portal on demand. any leads how can I achieve this.
