I was trying to embed the survey in MS Outlook and I found below threads regarding the same.
Embed surveys in Microsoft Outlook - ServiceNow Community
Embed surveys in Microsoft Outlook - ServiceNow Community
Plugin: Outlook Actionable Messages (App id: sn_ms_oam) is installed
But I was not able to see the option to enable Outlook Actionable Message check box in step 6 in my instance.
I have checked in Dictionary level as well, field is not there in that table. I see Outlook Actionable Message check box is showing up in Survey Designer. But when I enable and save the survey, again it is disabling.

I am facing the same issue in my instances. Did you ever get a reply back from ServiceNow support?

We had a similar issue which seemed to stem from the field not being present on the Assessment Metric Type Table. This was resolved by repairing the Assessment (com.snc.assessment_core) and
Assessment Designer Common (com.glide.assessment_designer.common) plugins. After repair the fields was present as expected on the table and the checkbox was present and visible on the Survey Definition.
