How to get VA to fill out the an incidet or request ticket

Good Morning Now Community, 

 

I'm a new sys admin and having the hardest time with trying to get the VA to fill out the Incident Fields for our Configuration items, Category, Sub-Category, and Assignment group. Same thing for request in getting the Configurtion item and Assignment group filled.

Here is the script and flow I've tried using:
