Standard change template not added to the Custom created Category after approval

Hi All,
I am facing issue for the Standard change template and its not showing up while I am trying to create change from portal along with the category I considered the template with.
Steps -
1) Create a Standard Change Template with Service Catalog > Standard Changes > Template management > Propose a new Standard Change Template
2) Add details and Request Approval
3) Once approved, the template should be available over the portal with the category selected.
4) When checked the category, the template even not added to the list.
Could anyone suggest or provide any clue.
