Flow Designer - Generation of tasks

Flow designer newbie here
I've built a flow in the flow designer.
When I test the flow, all tasks appear to be generating during the test.
However, when I activate the flow and submit the catalog item, only 1 task is generating instead of 2.
Trying to figure out why this would be happening.
Appreciate if someone can help.
