Need Help Creating Records for Each Updated Incident Field Using Flow Designer

I need some guidance on implementing the following requirement using Flow Designer.
Whenever an Incident is updated (for example, State, Assignment Group, or Description), I need to create a separate record in a custom table (xyz) for each field that changed.
For example, if the following three fields are updated in a single Incident update:
- State
- Assignment Group
- Description
Then 3 records should be created in the xyz table, with the following details captured for each changed field:
- Field Name
- Old Value
- New Value
- Updated By
- Updated Time
I am currently trying to achieve this using Flow Designer with the following setup:
- Trigger: Incident Created or Updated
- Using the Changed Fields object available in the trigger
However, I am not sure how to use the Changed Fields collection to iterate through all modified fields and create separate records with the old and new values.
Has anyone implemented a similar requirement using Flow Designer? Any guidance or examples would be greatly appreciated.
