what type of fields we can add to the CMDB Completeness Recommended score card. (eg. IP address, name, model ID, serial number, and company all fields are discovered via discovery, and on the form level we have 5 different fields that are recommended; some fields are non-discoverable but it's mandatory on the server, database, application class)

to the best of my knowledge, you cannot add any additional fields, as these scores are determined individually and collected in individual tables.

Field addition is not possible here and if you want to add, you need to change the Type of report.

"Limit the number of required fields, and use required fields that are automatically populated (e.g., using Discovery or SCCM). Limiting the number of required fields reduces the risk that CIs identified through Discovery will be blocked. As required, fields are mandatory across all areas of the CMDB while recommended fields only impact health metrics, use recommended fields to inform health metrics without impacting other processes and to test potential future required fields. "

The fields for a particular CI class that are used to store important data attributes can be considered as recommended fields. At the same time, these fields should NOT be marked as mandatory for that CI class (Since those will be considered as the 'required' fields). Examples of some fields that can be considered as recommended fields are Location, Manufacturer, Model ID, Support group etc. i.e. The objective is to consider the fields which might help the operation or support teams in speeding up tasks like root cause analysis of incidents/problems, impact analysis of incidents/changes, issue resolution etc. as recommended fields. The exact list of recommended fields for each CI class should be determined after discussing with configuration manager and respective CI class owners. It would be better to consider a limited number of important fields for each CI class as recommended fields.
Following is the procedure to set recommended fields:
Go to CI class manager, open a CI class and click on 'Completeness' tab under 'Health' menu. Then click on the 'Recommended fields' tab. You can set the list of recommended fields here (screenshot attached for reference).
