We have a location based support model and the location determines which support group is assigned to a CI. We have hundreds of locations and probably about 20 different support groups. However each of these CIs could also have a different managed by group. What would be the best way to assign support group/managed by group based on the location? Should we try to create service offerings per location? Should we create a custom mapping table for location to group?

You will always in my opinion, have service offerings for each resolver group :-).
So I would recommend to create technical service offering - using dynamic ci group, that selects all ci's based on location - and then populate the support / managed by groups from technical service offering - through ci class manager or built-in synchronize offering groups with CI's..
