Hello,

I was able to see the survey without logging in.

Are you using the same URL as below:-

https://instancename.service-now.com/sp?id=public_survey&type_id=sysidofsurvey.

If yes just check if the widget Surveys and Assessments public checkbox set to true:-

And also if the page with id public_survey has the public checkbox set to true:-

Hello,

No once you have completed the survey you cannot take same survey again by design . You will need to generate another instance of the survey for that.
