UI Action not showing up in the Agent Workspace even after enabling the Workspace Form Menu checkbox

Workspace Menu is enabled and the UI Action is active but still this doesn't show up in the Agent Workspace Menu.

I am facing the issue even after adding an entry in the 'sys_ux_form_action'
