assessment notification not working

Hello...

I'm a newbie on servicenow. I have problem with assessment notification.

I have generate assessment for vendor, but user didnt recieve notification. it working in my formarly instance. but not in my new instance. i'm using default assessment notification by servicenow. Did i missed something? i didnt do any change for that assessment.

Please help me?

just remove below condition from Business Rule and see if that helps,

In the when to send section of your notification, below groups and exclude delegates feilds(right hand side), there is send to event creator. Please check it and try once.

 I have the same issue regarding Assessment.

But my question is i cannot see the evaluation method attribute on my form as per your screenshot?

so it is OOB option to add or how can i add that attribute to send notification to a user?

i cannot assessment form to add more options as a business rule says evaluation method ?

 

Please give me some inputs on how to add those attributes and send email notifications?
