Blank screen for survey

Hi All

We have a survey which is sent to the user via email.

Once the user clicks the link to survey the survey page loads but is appearing blanks if you refresh the page the survey appears.

This is not a new survey and has been working as expected since going live in Jan.  

Any suggestions / advise on what could be wrong?

Could you check if the User who clicks on the survey link is an Active user.

Our Guest user was deactivated on the sys_user table
