Please follow the below steps to create new Standard Change Templates:
1) You can Create a New Category under Standard Change Category by navigating to "Maintain Categories" module under Service Catalog Application menu and then search Title ="Standard Changes" . Open the Searched record and create a new Category as shown below:
Once the new Category has been created the same is available under "Standard Change Catalog" module as shown below:
If you just want to add a new template under the existing OOB category available Follow the below mentioned steps:
2) To add a new template under any of the Categories as in the screen shot above,
a) Click on "Template Management" as shown below:
b) Then click on "Propose a new Standard Change Template" as shown below:
Note: Even the existing templates available under different Categories like "Network Standard Changes" can be modified by clicking on the link "Modify a Standard Change Template"
c) Fill in the fields on the Template form like short description and select the Category under which you want this template to add like Network Standard Changes or Database Standard Changes as shown below:
d) Once the required fields have been filled in Click on "Request Approval" button on the form as shown below:
e) Once the User clicks on "Request Approval" button a standard change proposal gets generated waiting for the Approval from Change Manger(Users with change manager) role as shown below:
Proposal can also be viewed under "My Proposal" module under Change Menu.
f) Once the Change Manager approves the Proposal request the template under the required category gets created as shown below:

Hello Shloke,
Once we send the standard change for proposal (when we click "Request Approval" button on a standard template to propose it) it autopopulates the assignement group to "change management" I was unable to find the exact script/ configuration where this group is defined.
Please help me with the above query! Thanks
