Fields not showing on Portal

I have a request that the only field that shows on the portal until a selection is made.  The field is Request Type, field choices are, New, Modify, Retire.

Depending on what you select different fields and visible and mandatory. My issue is if you select new, then change to Modify then change to Retire and go back to one of the other choices.  The fields are not there.  I have to reload the form to get the fields to show up.

I have the following UI policies.

8 UI policies on the portal.  I'm assuming that the issue is with one of my policies, but I can’t figure out which one

1 policy for each type that has the fields and field requirements, like mandatory.

3 Clear on Portal for each type.  This is to clear the values if the user changes their mind and selects a different type.

1 policy that hides 2 field sections that are only on the tasks and RITM

1 policy that makes an additional field required based on the answer of an approval field.

As you can see from my screen shot, after I click retire and went back to new, most of my fields aren’t there.

Everything is working fine other then if you select one of the request types and then go back.

I only attached screen shots of the UI policy for new, but the modify and retire are built the same.

Any advice would be greatly appreciated.

I think this has to do with the On Load functionality being checked. Try unchecking the box on the UI Policy for New.
