Help in creating GMUD table

I have to make a GMUD application, but I'm confused about how to create the table and what fields I put in, and what these fields will be like, these are the application requirements:

• Create a new GMUD request
• Must contain a description of the change
• It must contain the list of approvals for this change
• It must contain the date of execution of the change
• Must contain change status control
• Must allow gmud to be sent to different execution and approval queues

Does anyone have any ideas on how to do it?

Yep, that's Change Management.  Looks like either you do not have access to all the modules or they were disabled in your instance.

I'm using the free Dev instance, that might be why
