Based on documents and examples,
1.Create a catalog item
2. Create a trigger with Service Catalog in Flow Designer
3. Select the flow created in step 2 in the catalog item process engine.
I did this work.
However, the flow does not run when I request a catalog item.
What are some possible causes?
Set trigger on Service Catalog and create flow
Select flow in Catalog Item

have you checked the executions tab for any execution logs of this flow ? Also, can you try testing it using the Test button available.

@nakamura323 Your flow is not active, the activate button becomes disabled the moment a flow becomes active. In your case it is not disabled which is giving me an impression that the flow is not active.

Please check few steps-
1) Make sure you have activated/published your flow.
2) Test your flow in Flow Designer.
3) Attached your latest/published flow to catalog item.
4) Check flow executions.

It may be due to flow is not active / saved and also please try to map catalog item to category.
