Hi @Ralton Stewart ,
According to your screenshot, I can see that the incident is created from a record producer from the service portal.
To capture the different metrics that you're requesting:
1. Create a report based in the incident table
2. In the configure sections chose select columns:
3. In the popup select questions
4. Select the record producer of your choice
5. select the variables that you would like to add to the report.

Hey I had a look at your suggestion but I am not seeing the fields required unless I am doing something wrong see attached.

When you click the record producer of your choice, doesn't you get the available questions from the record producer in the available column?
