@Purushotham Ga2  you can skip the Template creation and use the existing one. In survey Designer you can use the existing Template like "Smiley Faces" you will get the same output as below,

Here are instructions on how to add an Emoji response to a Survey/Assessment question.

1. Download emoji files to your computer or accessible file location

2. In the Survey Designer, drag the "Image Scale" control to the canvas.

3. From the control properties, scroll down to the "Image Choices" section.

4. Select the ">" icon to view the expanded properties for each response and Upload the image of your choice.
