Hi,
You need an admin access, else you cannot debug this. Either contact your System Admin account user to debug on your behalf or if you get the Admin access, Navigate to "All" Module under System Logs Application Menu to check the log only if it's a very recent activity else I doubt you won't be getting this on logs also.
Also can you navigate to this Incident form and Right click on the header and select "History-->List" as shown below:
Post click on this, search for "Opened By & Opened Field" as shown below:
Open these Records and check User Name , Old Value and New Value,Update Number which will give you the details if these fields has ever been updated as shown below:
Note: But for this also I guess you would require admin credentials, you cannot debug without admin Role.

Hello,
Opened date is something when you first load the form and it will auto populate the date but when you submit the form first time then only it populate the created date, that's why it will not be same always.
If you see in Opened dictionary, the opened date takes the current date/time when you load the form first time.

FYI, please see highlighted text below there.
I under stand that "Opened date is something when you first load the form and it will auto populate the date but when you submit the form first time then only it populate the created date, that's why it will not be same always." In this scenario difference should in minutes.
But here there is difference of 2015-07-23 to 2017-06-06 (2 years)

Wow (2 years), i didn't notice the year. anyways, In that case can you please check if there are any Business Rule running which is updating the created date by any chance. Is it happening to all the incidents or few? May be you can try to find the pattern to understand the situation.

not all incidents but many please the below screen shot filter i given.
as it is OOB you can also check business rule could you please check if you get any clue.
Also can you please give the same filter in your instance and let me know if you are also getting like this records.
