Reset incident form to OOB

How do I reset the incident form to OOB?   I want to be able to see the subcategory fields again along with the options to select with the issue is.

When you nav to form layout for the incident form, you should see a related link for "Show Versions". click that.

Find the record that has the OOTB source of the upgrade. SHould look like something below. Right click on the version you want to revert back to.

How come my subcategory shows up as a string?   How can I get it like the category?


Cat and Sub cat are string fields OOTB. You just add a choice value to them.   With your screenshot, looks like you added tree picker to your custom category field.
