Error in My Tasks HRM Todos Summary widget

We are seeing below error for some of the users. This is observed for mostly for users who are having sc_req_item approvals in My Tasks. We have cloned the HRM Todos Summary widget.
Please help on this.

Hey @Janu sree -
Was this error happening before you cloned the widget, or, is it only since the widget being cloned?
I cannot remember off the top of my head if this type of error message from a cloned widget could be from a possible Restricted Caller Access (RCA) request. But, have you checked those to see if there is a record from the RCA list that is either invalidated or being requested since you have cloned the widget?

We didn't observed whether this error occurred before cloning the widget or not. Also there are no RCA invalidated records.
As per my few debugging steps like below-
1)I have reverted it to OOB widget(HRM Todo's Summary) on the hrm todos page, we are still seeing the error.
2)Then I felt there might be chance of role issue for the approver, As I'm having the admin role, I've assigned the RITM approval under my name, then the error is not visible also I'm able to see the approval task in my profile, when assigning back it to the reporting user(PFA roles he is having), then the error is coming back. So I added the admin role to the user. Then there is no error and the task is visible.
So seems like its role issue? but again I'm not sure how this is happening? Thanks for any of your inputs.
Current roles user is having
