Is there a way to include more application information on an application assessment?

Our implementation partner set up a basic assessment for us in APM that matched what we were doing before we adopted ServiceNow's APM. When I generated the assessments, the only thing that the assessor sees is the name of the application and then the questions from each metric below that. Is there a way to make it include more information about the application (like the description)?  Or even a way to include a link to the application record?  I know that I'm going to have owners who get these assessments need more information to help them understand which application it's referring to...

Hello @MEBoatman - I do not see any direct option available to customize this assessment UI page of APM. I see these two scriptincludes are responsible for populating the value in this assessment. Maybe you need to investigate for a few hours to decode and add the description of your application. Have a try and meanwhile, I too try and see.

scriptincludes name - APMAssessmentsUtil

Note: I tried creating a new template for this assessment, but it is not working. NPS template - scoring value does not have any option to updated the description directly.
