Questionnaire within audit that can't be standardized

Hello,

I have following requirement and it is really hard to come up with any possible solution or even a nice workaround for this:

My customer performs audits in ServiceNow by making use of audit engagement record functionalities. During these audits there is going to be one or more questionnaires answered (as an excel sheet) outside of ServiceNow. This questionnaire can't be standardized, which means the questions might be different in the future (and that also means we can't store any template or automized logic in this case).

In order to document this questionnaire we would make use of an audit task (derived from task table). The assignee for this task will be the person who raises the questions. It will be also his responsibility to transfer the questions and answers, because it might be the case that the question taker has no access to the ServiceNow instance.

To simply attach the excel-file to the task is not an option, because the customer wants to have the possibility to report on the questions and answers and wants to have everything documented in ServiceNow (fair enough).

So I was looking into the capabilities of the assessment feature in ServiceNow. But there are a couple issues I am facing there: The audit task assignees might not be very experienced with ServiceNow, so you can't expect them to create an assessment and fill it out on their own again and somehow link it to the task. But am I overlooking something here? I'm not too familiar with surveys and assessments in SN.

In conclusion I also want to add that I don't want to create more than one or two custom fields on the audit task table. Also they use the audit workspace as main UI. I actually don't want to set anything up in the ESC or somewhere else (but if you have a really cool idea that helps, feel free to suggest!).

This is a common pain area even i have faced in my implementations.

with the audit tasks functionality you cannot have questions or answers recorded and reported unfortunately. The most which you can do is attach as excel.

However, you can include a manual activity to convert these questions as assessment questions and report from assessment table, although it's not automating anything.
