Hi Everyone,
Just want to ask on how to modify the "Take Attestation" form screenshot below.
I tried to follow the instructions in How to Expose additional fields on the Attestation - ServiceNow Community and Solved: Show fields for an attestation - ServiceNow Community but it seems that it doesn't work on my end.
See the configuration that I have done below.
are there any options that I need to enable to modify this form?

You might have to use "Script" section , sample :
and if you want to modify or add new questions then:
What you can try is navigate to asmt_metric_type.list > Look for the required metric type>
Modify accordingly:
