What is the difference? (Evidence Request Form and Evidence Collection Detail Form)

Hi SNOW GRC Community,
Our company is trying to implement the evidence request module to facilitate collection and review of documents requested as part of audits and regulatory exams. We are currently collecting documentation manually and just using SNOW GRC as a repository once the documents have been collected. We do have plans to use the functionality directly in SNOW GRC but not at that point yet.
I was reviewing the following product documentation page on how to use this module and had a question related to the two forms that we need to fill out.
What is the difference between the "Evidence Request Form" and "Evidence Collection Detail Form"? Are both forms needed for the business need I described? Understanding the purpose of the two forms will help us provide clear instructions to our users. Currently, the users are confused because it looks like every request as two forms associated with it, and appears duplicative.

Hi
Both "Evidence Request Form" and "Evidence Collection Detail Form" are the same form, it's not different!!
Once you Create a Evidence Request using "Evidence Request Form" , you need to Create Evidence Request task to fulfill your Evidence Request, so that task is created using "Evidence Collection Detail Form".
So both are tracked like below explanations.
In the Evidence Collection Detail Form, once you have filled the form and save it then you get to see "Evidence Collection Detail Form" Populating in the related list tab, then you click "new" to fill the form.

How do you activate the Evidence Request table? It is not available in my instance and I do not see a plugin for it. The product documentation makes it seem that it should automatically be installed with Audit, but that is not the case for me.
