GlideModal upload attachment

Now I need a popup window to pop up before submit and ask the user to fill in the fields, some of which are required to upload attachments, I use GlideModal to call the UI page, in the UI page, how should I write code so that the attachments can be uploaded in the popup?

Thanks for your reply, but I still don't know how to create an attachment field in UI page and upload it to sys_attachment table.
