comments_and_work_notes not displaying Additional comments or Work Notes

Hi SN community.

We have a Form View for HR Cases where we want to just show the "comments_and_work_notes" and not the Activity Filter. All comments and work notes show correctly in Activity Filter.

But in the View where comments_and_work_notes is on the form they are not showing correctly, currently not displaying Additional comments and/or Work notes. (As shown in screenshot). 
Sometimes it is showing first Additional comment (like in screenshot, first additional comment), but then not showing the following (empty in screenshot). 
Sometimes it is happening to specific Users, and not to others. I have not found a pattern of roles or anything missing from Users where this is happening.
I have looked Client Scripts and UI Policies and have not found what is causing this.
I can also see in List view that the comments_and_work_notes field is getting populated correctly, it is just not showing them for some reason.

Can this be an ACL thing ? (Although we have not changed anything recently)

Can I ask the business use case for only showing comments and work notes and not all the other activities?  You can configure what appears in the activities for the COE types, and in Agent Workspace (UI Builder version), the user can filter/select what they want to see.
