Here are the high-level steps. Be sure the PDF document you will upload has fillable fields on the document. Once you have this file, follow these steps:

  1. Create the Managed Document record
  2. Create a revision of the Managed document to upload your PDF Documents that contain fillable fields.
  3. Publish the revision
  4. Create the PDF Document Template and select the revision uploaded in the earlier step.
  5. Parse the fillable fields to allow the data to be mapped to a table within ServiceNow. (After saving the new PDF Document Template you will see the "Parse PDF" link to facilitate this).
  6. Map your PDF fields to the fields within the ServiceNow table (typically the HR Profile or HR Case fields) 
  7. Mark Signatures on the Fillable PDF (If using Signatures)
  8. Once the PDF Document is saved you can add the PDF Document Template to a HR Case Template within a HR Service. This will allow you to create a Service Activity (HR Task) to present the Fillable PDF to an employee to sign the document.

Why would I want to use a PDF Document template rather than a Document Template? When I head to HR Administration -> Document Templates -> and click New I have the option to create a Document Template or a PDF Document Template. I've read the documentation, but it's not clear to me in laymen's terms what the difference between the two options is.

In my example I was using a fillable PDF so you have the ability to use these types of documents instead of creating document templates from scratch. Which document you use depends on your use case.
