Hi,

This is OOB. you can trigger survey using trigger condition and you need to check OOB notification for survey.

Hi, 

Are you trying to fill survey or configure the CSAT survey.

ServiceNow has OOTB Survey module, you can configure the Survey questions based on your requirement.

 

Refer the below :

For Example : Open 'Customer Satisfaction Survey' and review all details, you can review other Survey definition and replicate. Survey will trigger based on trigger condition define on said table record [ like incident or task '

Hi Ashish,

Follow the below steps to create a survey.

1)Go to the View survey and create a new survey with name "Service Desk Satisfaction Survey".

2)Give the condition when to send the survey.

In the related list of that survey you will find the Trigger condition related list.Create a new trigger condition using condition builder as State changes to closed or state is closed.As shown in below image.find_real_file.png

3) Click on the Survey Designer to create a survey questionnaire as per your choice.save and preview the same as shown in below image.you can create a dependency while creating questionnaires. find_real_file.png

4)You can check survey that has been sent in Assessment instance related list.
