Could someone please help with a template which can be used to gather inputs from SMEs to understand the 'edges' of each applications and related config file locations to trace connections.

I think You Have Service Mapping.
Why do not you do reverse- Run Service Mapping on basis of Traffic Based discovery . It will bring lot of data and then get it verified by SMEs and Trim those excess Cis from Map..

Use traffic-based discovery at the initial stages of discovering an application service and disable it once you completed discovery and fine-tuned the application service. You can choose to hide CIs discovered using this method from an application service map. In that case, these CIs are still part of the application service, but they are not on its map.

There is no universal template that will fit all your needs for every single app.
However, you can definitely start with some questions that apply to every single case:
- What is an entry point(s)?
- Who is the owner of an application?
- SME(s)?
- Name for the app (the name is editable, you can change it later if you need to)?
- Base components the owner and/or SME expected to see like: VIP, LB, Web Server, App Server, Database.
Once you have these answers you can use approach described by Rahul.

Agreed,this is at a very high level. Iam looking for a template to shared with the SME. For Eg : something like below.
- Architecture of the applications.
- Application config file location.
- Database connection config file.
- ...

Yes,this simplifies the initial draft and reduces all the effort in customizing patterns but not sure how well this approach works for a dynamic environment where I have number of new servers spinning up every day.
