Catalog items requests on Service Portal are not submitting.

The same catalog item works when I click 'Try it' but it freezes and doesn't submit on Service Portal.

This is the error message I see when I inspect the page:

I made the Catalog UI Policy inactive and it worked. Don't know what is the issue.

CAn you eloborate more on this , 

We recently moved all the Service Catalog items to Service Portal and there is a problem in submitting some forms and when I click 'Inspect' on the web page, I find this error. I'm not able to find the reason behind this.

Have you created a new catalog item for the portal to ensure this works as required?

Also - can you put up the configuration of your catalog item, it may have some config that isn't supported in the Portal.

It doesn't have any client scripts, just a UI policy.
