How to give the heading before questions in survey's?

Promoter Score

1 (Very Dissatisfied)

2 (Dissatisfied)

3 (Neutral)

4 (Satisfied)

5 (Very Satisfied)

How to give the heading before questions in survey's?

Can anyone please help on thuis, It will bne usefull.

Would you like to add the headings as highlighted below?

If yes, then they are nothing but a section name. Open your survey in the survey designer and add section names as required.

I tried this but i need to see in the survey form like below one by one questions in one page but it is showing one question if i click next button it is go to next question and if i preview it is showing one by one in one page(all question in one page).

you can configure that in your survey via survey designer

Open that survey from the backend and select pagination setting as None. PFB
