Priority field on incident form.

In our PDI on Incident form the Priority field is auto populated to 5 - Planning based on impact and urgency field's value while opening the form for first time to create new Incident.
I want choice values of Priority fields should be 1 - Critical, 2 - High, 3 - Moderate, 4 - Low and 5 - Planning should not be auto populated while opening the form for first time to create new Incident.
I have inactivated the choice value 5-planning and priority lookup for choice value 5-planning as well but still it is showing while opening the form to create new incident.
Can anyone please help me out ASAP as it is urgent.

So what do you want the priority to be set to based on creating a new incident? Or do you want to leave it blank?

So when should the priority of the incident be set? How time should the priority be blank?

Hi @abhisek ,
Just go into form design and change to "dropdown with none" for urgency and impact:
And change the default to "none" for urgency, impact and priority
Afterwards, go into "priority lookup rules"
add the none
Now when you create an incident it will state:
