Stumped on adding HINTS on the Change Record

Hello,

 I have a task to add HINTS to be displayed on every selection made on each choice made on the Reason field on our Change record. 

These hints will be shown/displayed for every selection, below the Reason field.

I'm stumped on how to achieve that.
