What is the table name of incident survey in service now?

I want to create a report for survey for incident. so what is the table name?

Hi,

Survey does not specific to one table (like incident). The table for the survey is assessment_take2.

You can find the related survey that has been done on assessment table.

All surveys in servicenow are present in "asmt_metric_type" table
