Workflow Not Getting Attached RITM For Any Catalog Item

Hi All,
I have an issue where my workflow is not getting attached to my RITM. On further investigation, I could find out that this is not happening for just one catalog item but for every catalog item.
I also went ahead and checked my Request. There I can see that my Request Workflow is as below :

Can anyone please help me understand as to why my workflow is not getting attached.

That's your request workflow. go to RITM and click on show workflow.

The RITM workflow won't kick off until the parent request workflow (shown in your screen shot) completes and the parent request is approved.

Hi Stephen,
I am not sure as to why my Request workflow is not getting completed.
Can you please help me debug the same.
