Workflow Not Getting Attached RITM For Any Catalog Item

Hi All,
I have an issue where my workflow is not getting attached to my RITM. On further investigation, I could find out that this is not happening for just one catalog item but for every catalog item.
I also went ahead and checked my Request. There I can see that my Request Workflow is as below :

Can anyone please help me understand as to why my workflow is not getting attached.

Can you share details about the active workflow behind the Request? Also hover over the step the workflow is pending on, what does it show?

Looks like its stuck at the if condition of the REQUEST workflow.
Can you share what's in the if condition script.

Hello Mark,
That's the issue I am facing. Workflows are not getting attached to my requests or RITMs. And this is happening not just for one Catalog item, but for every catalog item in my system.
