UI action record skipped after instance upgrade

Hi All,
UI action record was skipped after instance upgrade. And I can not click on it (Not accessible - its seems it is greyed out) How to resolve this issue.

Try following thing.
1. Ensure you are in correct scope
2. Open the browser in incognito mode or user private browser and check
3. Check the difference between your existing UI action (customization) with base UI action
4. Upgrades can sometimes introduce new ACLs that restrict access to system tables . Check if that happened at your end.

Hi @Obito,
This happens in ServiceNow after an upgrade when your UI Action has a conflict with the new out-of-box version. The system marks it as a “skipped record," which is why it appears greyed out and not clickable.
To resolve it, go to Upgrade History, Skipped Changes, open the skipped record, and compare your version with the new one. Then you can either accept the new update or retain your customization based on your requirement.
