Error Message on Resolution notes generation GlideModal-Now Assist

Steps to Reproduce:
Step 1: Open any incident in In Progress state, View: Default
Step 2: Click on Resolve Button.
Step 3: Pop up (Glide Dialog Box) will open which will generate resolution notes.
Error Message: "Form Control g_scratchpad.trigger_field (.cmdb_ci) not reachable"
Above error goes away in some seconds, but it should not even come. This is coming from some OOTB script.
Please suggest solution to get rid of the above issue.

Is this occurring on the Incident record or the Change record?
You statement says it's incident but your screen shot is from Change request.

It is on Incident form, and the screenshot is also from Incident form. We haven't activated the skill for Change yet.
