How to add record producer variables in the reports directly

Dear all,

Now I am creating reports for the record producer form. 

Before this, I have been mapping the variables of the record producer to the table, and then the reports use the table data.
But now I need to use reports to record the checkbox-type data in the form. I don't think it's worth creating more fields in the table just for a single item. So I want to know how to add record producer variables in the reports directly.

You can report on incident table & then add Questions accordingly. Something as below.
