Activity Stream in workspace differs from platform activity stream

One of our customer is experiencing issue with activity stream in workspace. The order of events shown in activity stream in workspace is different from the activity stream in platform. Moreover, somehow it clubs or changes the activity stream which gives wrong info. Attaching the screenshots of the same.

You can see that there is a difference in the activity stream in both the places. Moreover the activity stream in workspace gives wrong info as it shows in a message that the record was assigned from Irene to Javier at first which is not correct. 
As per the customer the issue is intermittent and they are using custom flows.

Any help is appreciated.

      Are you able to troubleshoot the issue. I am also facing same issue in agent workspace when compared to platform
