Add process flow formatter to form

How can I add a 'flow formatter' to the top of my form?

I have already turned on the plugin, created the different stages and ordered them in the 'process flow UI'.  

Please advise how to 'configure' the form to add the 'flow formatter' at the top.  Thank you

Hi,

Configure the form layout and select the "Process Flow" from the slush bucket and move it to the top.

Configure > Form Design

Put the process formatter on the top of the page in it's own container

It will appear in the Fields > Formatters section at the bottom
